What is the phieu ly lich tu phap so 2?
The phiếu lý lịch tư pháp is the judicial record extract issued by the Vietnamese Ministry of Justice or by the provincial department of justice. It exists in two forms that do not say the same thing. No. 1 states only convictions whose expunction has not yet been granted: someone whose conviction has been expunged appears on it as having no record. No. 2 states every conviction, expunged or not, along with any prohibitions on practising. That is why IRCC names no. 2 within the very name of the document it requires, and why no. 2 may be requested only by the person concerned or by a judicial authority — never by an employer.

How to obtain the document in Vietnam

Ask explicitly for số 2
Write it on the form and say it at the counter. No. 1 is the default administrative reflex, because it is what the Vietnamese labour market asks for. Check the number on the document before you leave.

Go to the right department
The request is made to the department of justice of the province where your residence is registered, or to the national criminal records centre depending on your situation. From abroad, it goes through a Vietnamese diplomatic mission.

Because no. 2 is far more detailed, its issuance to a proxy is more tightly framed than no. 1. Check the conditions before entrusting the process to a relative back home.

What an acceptable translation requires
IRCC requires any document that is not in English or French to be accompanied by a translation into one of those two languages. The translation must be complete — headings, phiếu number, conviction or no-conviction entries, stamps and signatures included — and may not be produced by the applicant, by a family member, or by a representative on the application. The number matters especially here: it is what lets the officer check that you have indeed provided no. 2. The pitfalls specific to this document
No. 1 will be refused, and the whole process is lost
The two documents look alike: same heading, same layout, one digit apart. But no. 1 hides expunged convictions, and IRCC names no. 2 within the very name of the document. An application filed with no. 1 comes back, and you start over from Canada.
A "no record" entry on a no. 1 means nothing to IRCC
On a no. 1, that entry means only that no unexpunged conviction remains. It does not rule out an expunged conviction, which no. 2 would have shown. That is exactly the information an officer is looking for.
Vietnamese diacritics are not decorative
Nguyễn, Nguyên and Nguyện are three different names. A transcription that flattens them creates a gap with your passport. The translation keeps the Vietnamese form and aligns the Latin version with your passport.
The document has a short shelf life
Vietnamese and foreign bodies alike generally treat a judicial record as valid for six months. Requesting it too early in an application that then drags on means requesting it again — from Canada, that costs weeks.
